Usually you leak it from your ears. But that's a bit too serious an injury for Lestrade to recover from quickly.

My only guess in regards to Sherlock's failing to notice John not being there is that he's spent so many years on his own, talking to himself, that he's not in the practice of making sure someone is listening or not. I mean, he used to talk to the skull. It didn't matter if the skull answered back.
