By "both," do you mean Sherlock and Tzophiya, or Sherlock and John? Perhaps they are all in denial. I recently rewatched "Hounds" (Not-So-Small Child's first viewing of it) and again boggled at John's low-key reaction to discovering that Sherlock experimented on him and kept him locked in a lab to observe his reaction.
It's not that Ella's a bad therapist for John; I think no therapist could really be adequate for that man. And Sherlock is worse.
