I liked the episode overall as well. Much more than I thought I would.
As for the layout of 221b, my understanding is Mrs Hudson is on the same floor as "Speedys" and the entrance. Sherlock's bedroom, the kitchen and sitting room (and toilet I think) are all the floor above that and john's bedroom is just above them and it's the only think up there
